Intensive Case Managers (ICM) $1,500 Sign-On Bonus! Vineland, Clifton, Somerville

The role of the Intensive Case Manager is to make a positive impact on the lives of the GA and TANF population who face many challenges.

As an Intensive Case Manager, you will play a vital role in supporting individuals with complex health and other needs. You will work closely with clients to develop individualized plans of care, connect clients with essential resources, and advocate for their well-being. This position requires a high level of dedication, compassion, and collaboration as you navigate the complexities of community systems. This is a non-clinical position.

-Conduct comprehensive Social Determinants of Needs (SDON) Screenings to identify clients’ needs, strengths, and challenges, and develop individualized care plans with clients.

-Maintain a caseload of 40-50 clients.

-Update monthly care plans and ensuring clients are attending their services goals.

-Connect clients with a wide range of resources, including housing, employment/job training resources, and community support services.

-Advocate for clients and ensure they receive appropriate care and services by eliminating barriers.

-Monitor clients’ progress, document interventions, and update care plans as needed.

-Collaborate with a multi-disciplinary team to ensure coordinated and comprehensive care

-Attend training and practice Mental Health First Aid (in the fall of 2024)

-Depending on risk status or need, provide weekly or bi-weekly contact with the clients.

-Educate and empower clients to increase their self-sufficiency and manage their symptoms effectively.

-Maintain accurate and confidential client records.

These ICM positions are located in the areas below, with an option for hybrid once fully trained.

-Based out of Cumberland (Vineland) with travel to Atlantic (Pleasantville) and Cape May (Rio Grande)

-Based out of Clifton with travel to Bergen and Hudson

-Based out of Somerset (Somerville) with travel to Hunterdon (Flemington)

QUALIFICATIONS:

-Minimum of 2-3 years of experience working with individuals with complex behavioral health needs.

-Certified Peer Recovery Specialists (CPRS) encouraged to apply.

-An understanding of mental health and substance use conditions, treatment modalities, and community resources.

-Excellent communication, collaboration, and advocacy skills.

-Ability to work independently and manage a diverse caseload.

-Culturally competent and sensitive to the needs of diverse populations.

-Proficient in Microsoft Office Suite and electronic health record systems.

Intake Coordinators $2,000 Sign-On Bonus! Bergen/Hudson, Somerset/Hunterdon, Camden, and Gloucester/Salem Counties

The Intake Coordinator conducts comprehensive substance use and mental health disorder assessments and then makes treatment referrals based on clinical need. These licensed clinicians will assess 30-35 clients monthly, with 3-4 assessments scheduled per day. The Intake Coordinator does not carry a caseload beyond the treatment referral. This role will require driving within a regional area to conduct in-person assessments. This licensed clinician is comfortable working with a complex and challenging population who have multiple needs and is driven to success through compassionate service delivery.

The Bergen/Hudson position is located in Rochelle Park with travel to Jersey City. The Gloucester/Salem position is located in Sewell with travel to Penns Grove. The Somerset position is located in Somerville with travel to Flemington. The Camden position is located in Cherry Hill with no travel.

A Master’s Degree in counseling, social work, or related field with an LPC, LCADC, or LCSW is required in order to provide telehealth services. Must have a minimum of two years of co-occurring disorder experience and applied knowledge of The ASAM Criteria, 3rd Ed., and the DSM-5-TR.

Referral Coordinators $1,500 Sign-On Bonus! Newark and Robbinsville

The Referral Coordinator is responsible for processing referrals, creating assessment schedules, answering phone calls, coordinating services and performing administrative tasks.

Must possess strong clerical skills and computer skills, including MS Word, Excel, Internet, and web-based operating systems with the ability to manage complex schedules. A minimum of two years work or volunteer service in the behavioral health care field preferred.
